My Family Tree
INSTRUCTIONS: Enter YOUR name and the name of your SPOUSE ( please use maiden name if known). Fill in the known names in your family tree. Leave blank any generations you do not know and skip to the generation you do know. If you know the name of your great grandfather, his father would be listed in the next group down as your 2nd great grandfather etc. In the boxes on the left list the mother and father and their date of birth etc. Below the parents name enter their relationship to you (great grandparents etc). In the boxes on the right list their child to whom you are related and their siblings. If possible list the children in order of birth starting with the oldest.
Print the form and mail to the family association with your membership application and dues. Be sure everything you enter is showing in the box. Words that are not showing will not be printed. If you prefer you can fill in the form after printing it.
In these boxes enter YOUR name and the name of your SPOUSE. Male (husband) should be entered in the upper box. Female (wife) should be entered in the lower box. Enter your children on the right.
